A geriatric care manager is a health and human 
services professional, such as a gerontologist, nurse, social worker, or psychologist with a specialized focus on issues related to aging and elder care. Geriatric care managers work privately with older adults and their families to create a plan of care that meets the needs
of the older adult. They will meet with you to help you understand your loved one's needs and to learn what resources and options are available to meet those needs.

  • Attend medical appointments and report summary of the visit to family.
  • Schedule and manage healthcare appointments by providing reminders and arrangement of transportation and prescription refills.
  • Act as a Liaison between the senior's healthcare providers and family. This can be provided even if families are long distant.  Although families my not provide the hands on, day to day care, we feel remaining well informed is essential to the care of your senior and your peace of mind.
  • Provide a comprehensive assessment of each individual’s specific needs, resources and limitations to create an individualized care plan.
  • Provide information and referrals to other geriatric professionals in the
    community.
  • Assist with locating alternative living options.
  • Assist with benefit applications and eligibility for services.
  • Educate seniors and their families so informed decisions can be made.
  •  Act as a point of contact to families going on vacation or those not
    easily accessible
  • Coordination of final life transition affairs such as referrals to funeral
    homes, acknowledgments of sympathy, consolidation of belongings, appropriate
    notification to creditors, etc.
